Scripts, templates and online consultation tools help staff collect consistent information and reduce improvisation under pressure.

These tools do not replace judgement about role boundaries. A template guides which questions to ask; it does not authorise non-clinical staff to interpret symptoms or to ignore escalation rules.

Use tools safely

  • Use the current approved version.
  • Do not skip required safety questions.
  • Do not add clinical labels that the patient did not use.
  • Do not change urgency because the system is busy.
  • Escalate if the answer does not fit the template.

A template supports safe information gathering; it is not a licence for non-clinical staff to triage around the system.

Scenario

A patient phones because they cannot complete an online request form. A colleague says, "Just put it through as routine because the form will not let us go any further."
