In a busy outpatient clinic a staff member suddenly realises they forgot to disinfect a tonometer prism between two patients. The practitioner stops, aware that an immediate containment decision and clear documentation are required.

What should happen immediately and how should it be documented?

During a particularly busy hour several patients used the same trial frames but staff later discover the frames were not cleaned between uses. The team must act to remove risk, restore safe flows, and record changes to prevent recurrence.

What controls and follow-up are appropriate?

  • Accountability details to record: who identified the lapse; what immediate steps were taken; when patients were informed if required; why workflow changes were chosen; and the date to review whether the fix worked.[3][4][2]
