Scenarios: Cleaning Lapses

Cleaning errors happen under time pressure. The goal is to contain risk quickly, learn, and prevent repeat lapses. Records should show proportionate action and patient protection.[3]
- Accountability details to record: who identified the lapse; what immediate steps were taken; when patients were informed if required; why workflow changes were chosen; and the date to review whether the fix worked.[3][4][2]
