Dementia Awareness

  • Dementia can affect memory, communication, orientation and the ability to manage appointments.
  • Do not assume every change in behaviour or thinking is dementia.
  • Use patience, simple clear wording and record any support needs.

Communication and Access

  • Give one instruction at a time and allow time for a response.
  • Check that reminders and appointment routes are accessible and usable for the person.
  • Repeated missed appointments may indicate problems with access rather than non-compliance.

Carers and Confidentiality

  • Carers often provide practical information about needs, routines and risks.
  • Confirm consent, proxy access or legal authority before sharing personal information.
  • Keep the patient involved in decisions and communications when they are able to take part.

Safety and Records

  • Sudden confusion, rapid decline or new risky behaviour should be escalated promptly.
  • Record practical support needs, reliable contact details and any agreed safety plans clearly.
  • Repeated access barriers should be reviewed and addressed as system issues where possible.
