Adult Safeguarding Scenarios

These scenarios apply the safeguarding guidance to realistic pharmacy situations. Consider possible coercion or abuse, listen for the adult's expressed wishes, be aware of capacity issues, and record facts accurately before escalating concerns.

Scenario

You are at the medicines counter when an older woman arrives with a neighbour to collect her dosette box. Each time you address the woman, the neighbour answers for her and says, "Don't confuse her, she doesn't understand any of this." The woman looks anxious, tries twice to speak, and reaches for the bag herself, but the neighbour takes it first. You notice fading bruises on the woman's wrist, and the neighbour insists they must leave immediately.

What should you do?

Scenario

You are making a home delivery to an 82-year-old regular patient with mobility problems. A man who says he is the patient's son opens the door, tries to take the medicines, and says his father is asleep. While you are speaking, the older man appears in the background looking unwashed and distressed and quietly says, "Please don't leave them with him, he keeps my money and I missed my tablets again." The son becomes annoyed and tells him to go back inside.

What actions are appropriate?

These scenarios show the importance of hearing the adult's voice, recognising possible coercion, recording facts carefully, and not making assumptions about capacity or consent.
