Scenario

You are working at the medicines counter on a busy after-school shift. A father arrives with his 6-year-old son to collect a repeat prescription for eczema cream and an inhaler. You recognise the family because there have been several recent collections for flare-ups. The child looks tired, is wearing thin dirty clothes on a cold day, and is scratching his skin until it bleeds. When you ask whether the cream has helped, the father laughs and says he keeps losing it and has not had time for the GP follow-up. The child stays silent and looks at his father before answering even simple questions.

You are on reception just before closing when a 14-year-old girl asks whether a prescription is ready. An older man waiting outside keeps watching through the glass and motions for her to hurry.
