Everyday hazards cause most harm. Simple controls and quick responses prevent incidents and reassure patients. The following scenarios show proportionate action and clear records.[1][7]

Scenario 1

An elderly patient is using the practice waiting room when they trip over a bag left on the floor. Staff and other patients nearby rush over; the patient is clearly shaken but no obvious serious injury is visible. The reception area is momentarily congested as staff offer assistance and check the immediate surroundings.

What immediate actions and records are required?

Scenario

During a routine eye examination a fundus camera becomes noticeably hot and emits a faint burning smell while in use. The clinician stops the examination and moves the patient to a safe area while colleagues inspect the device and the power connection. The device is in a clinical room with scheduled appointments awaiting resolution.
