Scenarios: Post‐Incident

After an incident, teams can fracture. These scenarios show how services are steadied while people are treated fairly and patient safety is preserved. The examples below reflect common workplace dynamics after allegations or investigations and focus on restoring professional working relationships while protecting staff and service users. ^[8]^[5]

Scenario 5 - Team fallout

Scenario

A harassment complaint has divided the team into recognisable "sides." Gossip spreads through shared spaces, informal conversations become accusatory, and rota swaps that were once collaborative are now hostile and transactional. Tensions threaten safe staffing arrangements and steady service delivery.

How can professionalism and safety be maintained?

Re‐state zero tolerance and confidentiality; discourage gossip in shared spaces and redirect concerns to formal routes. ^[1]^[2]
Adjust rotas to reduce friction and consider pairing or other controls to limit unsafe interactions. ^[1]^[5]
